Thorium-228 is a radioactive isotope. In irradiated thorium elements, 228Th can be produced through specific nuclear reactions. For example, when radium-226 is irradiated in the High Flux Isotope Reactor, actinium-227 and thorium-228 are produced. As a byproduct of the production of actinium-227, thorium-228 can be produced in large quantities.

Thorium-228 has a wide range of applications in the medical field. It can be used to make radium-224/lead-212 generators, which allow radium-224 extracted from thorium-228 to decay over time and produce lead-212 and bismuth-212. Lead-212 and bismuth-212 can be used to study targeted alpha therapies to attack metastatic skin cancer and neuroendocrine tumors with minimal damage to surrounding tissues. In addition, thorium-228 can be combined with other biomolecules to develop a cancer targeted therapy combination. In addition to medical applications, thorium-228 may have other potential applications. However, due to its radioactivity and decay characteristics, these applications need to be carefully evaluated and strictly managed.
